Pressed for time, Quoting or Calling it What it is?

Earlier this month — April 7th to be exact — I walked past a newspaper vending machine (Remember those?) near my house. There were a couple of big headlines on the front page, and above the fold, but one in particular caught my attention.

‘MALFUNCTION’
Navy jet crashes into apartments, pilots eject

What caught my attention was use of the word ‘malfunction’. Was the editor of the Daily Local News quoting a source, describing the incident or just pressed for time when drafting the headline?
